Elite FC OCL/Copl/npl (US Club) Carding info

Elite FC players participating in the Ohio Champions League will need to complete the following forms and provide a copy of their state certified birth certificate. Also make sure you provide a wallet sized picture of your player. Please bring all your completed forms, birth certificate, and picture to the first practice and give them to your parent rep.
